Introduction
To obtain benefits through First
Commonwealth’s Dental Network Plan, you and your dependents must select a
primary care general dentist office.
How
First Commonwealth Selects Dentists
All dentists that participate in the First
Commonwealth Dental Network Plan must meet their quality standards and participate
in an ongoing Quality Assessment Program.
Education, experience and dental office facilities are all evaluated so
that you and your dependents will have a selection of quality dentists.
How You Select a Dentist
Each covered family member may select a
different participating dental location. To
obtain information on the dentists in our network, you may call the Member
Services Department at (800) 239-3373, between 9:00 a.m. and 6:00 p.m. Eastern
Standard Time, Monday through Friday, or visit www.firstcommonwealth.net
to located participating dentists. Information
available about the dentist includes office hours, length of time in practice,
educational background and any foreign languages spoken in that office.
How to Enroll
Once
you and your dependents have made your selection, enter the dental location code
number(s) associated with the selection in the allocated space(s) on your
enrollment application.
How to Change Dentists
Each member may change dental locations at any point
during the benefit year. To do so,
please notify First Commonwealth of your intent to change locations by contacting
the Member Services Department. If
you notify First Commonwealth of your change by the 15th of the
month, it will become effective the first day of the following month.
