Dental Plans, through First Commonwealth
Over a thousand MAR members all across Michigan participate in these programs.
Blue Cross/Blue Care Network Medical and Prescription Coverage
You can search for network providers for Community Blue PPO plan by visiting www.bcbsm.com - go to “Find a MI Doctor ” (right side of the screen), select “Choose a plan - Community Blue”, select specialist, group, county and follow the prompts.
You can search for network providers for Blue Care Network by visiting www.mibcn.com - go to “Find a Doctor ” (right side of the screen), click "Blue Care Network" under "Select a Plan“ and follow the prompts.
All rates depend on the Michigan County in which you have your primary office, and your age. Regardless when you enroll, your rates will renew May 20 each year.
First Commonwealth/Guardian Dental Coverage
There are two dental coverage options available to MAR members, a DHMO plan and a PPO plan. You can view the benefit highlights of these plans (along with the quarterly rates), by clicking on “Dental Programs” above.
You can search for dental providers for both plans at www.guardianlife.com. Select “Resources” (top right of screen), click on “Provider Online Search”; click “Find a Dentist”, and follow the prompts.

We will be happy to provide you with a no-obligation proposal if you are interested in learning more about coverage options. Please complete the “Request for Proposal” form in its entirety, and either “Submit” it, or print the form, complete it and fax it to Group Associates at (248) 593-2065.
